Maryland Responds: Volunteer Opportunity Accountable Health Communities Screener
December 10, 2021The Baltimore City Health Department’s Accountable Health Communities (ACH) project is seeking an AHC Screener to assist in the success of the Accountable Health Communities project which is dedicated to improving the health of Baltimore City residents by addressing social determinants of health (I.e. food, housing, transportation, education, finances, etc.) in a clinical setting.
Volunteer Roles and Responsibilities: In this position, screeners will screen patients for various social needs and get patients connected to resources to improve their health outcomes. This position is especially suited for students or individuals that are interested in obtaining more experience working with patients in a clinical setting; and for those interested in social work, practicing medicine, and/or social determinants of health.
- Receives HIPPA training to ensure patient information remains protected.
- Screen patients to identify social needs either telephonically, or on-site at one of our clinical sites.
- Will receive training on how to properly screen patients utilizing the AHC screen.
- Take responsibility for contacting patients in a timely yet compassionate manner.
- Provide supplemental gap coverage for screening at clinical sites during peak holiday season from November through February.
- Data management which includes: organization of files containing patient data and information.
- Work with HealthCare Access Maryland community health workers to ensure patients are being navigated successfully.
- Have a baseline understanding of how social determinants of health can impact health outcomes in Baltimore City.
- Attend regular meetings to assess the efficiency and efficacy of screening processes.
- Any other duties that work to ensure the success of the AHC project.
Required Traits:
- Efficient in Microsoft Word and Excel.
- Is able to hold conversations over the phone with a variety of stakeholders including patients and community-based organizations.
- Demonstrates ability to read and write clearly and effectively.
- Demonstrates understanding of basic technology skills.
- Is a team player and possesses the ability to work well with others.
- Has a computer, phone, and access to the internet during work hours.

- Clinical setting experience and/or background in public health.
