March 6: Decreasing Emergency Department Readmission Rates Through Patient Outreach
February 19, 2019
UMMC Nursing Grand Rounds
Wednesday, March 6
2-3 p.m.
UMMC Auditorium
Repeat non-emergent visits to the emergency department (ED) for minor health care needs are not uncommon in an urban environment, where it can be more convenient for a parent to take children to the ED to be seen for acute needs rather than to their primary care site. Join us to recognize the impact of a multidisciplinary outreach team approach in decreasing repeat ED visits in a pediatric primary care population. Meghan D’Angelo, MSN, RN, nurse manager, and Casey Williams, BSN, RN, CPN, pediatric care coordinator, both at UM Pediatrics Midtown, will present. Light refreshments will be provided.
