The UMB HRPO has released a stand-alone research consent form to be used for all in-person contact research studies effective Monday, Sept. 21, 2020 at 1 p.m.

Please note that the form will need to be edited where prompted to include contact information and the signature line template as shown on the respective study's current approved consent form. Please contact the HRPO should you have any questions at HRPO@umaryland.edu.
