Let UMB Know via New Form if You Would Like to Be Considered for Vaccine

The University of Maryland, Baltimore (UMB) has been allotted more doses of COVID-19 vaccines for its employees and students. To schedule vaccinations as efficiently and equitably as possible, if you would like to be considered, please complete this new UMB vaccine form. The University will randomly select employees and students to schedule appointments.
The new form will let UMB know whether or not you want a vaccine from UMB and allows you to tell the University whether you have already been vaccinated, including reporting your dose information and any side effects, regardless of where you received the vaccine. You do not have to complete the new form if you have filled out previous forms. Vaccination dose and side effects can also be reported via the SAFE on Campus daily health screening.
UMB cannot guarantee that anyone at UMB will be vaccinated by virtue of completing the form. We want you to be prepared if the opportunity for vaccination becomes available. If you find that you have the opportunity to get your vaccine doses elsewhere, please do so. You can find information about Maryland’s vaccination phases plus how to sign up for the vaccine at various locations here. Even though Maryland is in Phase 2A and will enter Phase 2B on March 30, as a student, faculty, or staff member of a Maryland higher education institution, you will be prioritized ahead of Phase 2 patients when registering at any vaccination site in the state.
The more people who get vaccinated, the safer we will all be and the faster we can be rid of COVID-19. Once vaccinated, you still must abide by UMB's COVID-19 safety protocols and requirements.
