The Baltimore City Health Department (BCHD) is collaborating with all Baltimore City Health Systems to complete the 2023 Community Health Needs Assessment (CHNA). Assistance is needed from Baltimore City residents only.
The CHNA systematically gathers data on community needs and resources to more effectively direct efforts and improve public health outcomes. The assessment's findings will inform BCHD's 2024 Community Health Improvement Plan (CHIP), providing a roadmap to address identified needs over the next three to five years. This data-driven approach will guide priorities for both BCHD and the Local Health Improvement Coalition (LHIC).
Community input is essential to capture local insights and perspectives. Please share the CHNA Community Member Survey link with your networks so we may incorporate diverse feedback. This includes students, faculty, and staff. They must be Baltimore City residents to participate.
