You can approve or deny Chesapeake Regional Information System for our Patients COVID-19 health information sharing, upload your vaccination information, and request an exemption.


The University of Maryland, Baltimore (UMB) Recovery Task Force has introduced a new one-stop online shop for submitting COVID-19 information.

The UMB COVID-19 Management Portal allows faculty, staff, and students to:

  • Approve or deny Chesapeake Regional Information System for our Patients (CRISP) COVID-19 health information sharing
  • Upload vaccination dose information (if not using CRISP)
  • Schedule vaccination appointments at the UMB Vaccination Clinic
  • Request a vaccination exemption

The URLs for previous individual forms will redirect to the new portal, but it’s a good idea to bookmark the new link for quick access. Additional features could be added to the form in the future. Some information requested in SAFE on Campus will redirect users to the new portal.

Users will first see the CRISP authorization screen when they log in if they haven’t answered the form previously. CRISP collects COVID-19 vaccination information and testing results from connected providers in Maryland, Delaware, Virginia, West Virginia, and Washington, D.C., and select other states. If you authorize CRISP, most faculty, staff, and students will not need to manually submit testing and vaccination information.

Test results and dose information are typically shared the next day to the COVID-19 Management Portal via CRISP. Visit this Elm story on CRISP for more information.

UMB faculty, staff, and students are required to have their final vaccine dose by Aug. 2 for the fall semester.

The portal will let users know whether vaccination information is on file or is needed. If not using CRISP, users are provided an option to manually submit dose information.

If not currently vaccinated, users have the option to schedule an appointment directly through the portal at the UMB Vaccination Clinic.

An option will also display for those who are not vaccinated and wish to request a medical or religious exemption. That process will take users through a step-by-step process for electronic signatures, any required downloads, and submissions needed.
